31. Komplexe Bauchwand- und Narbenhernien

Zusammenfassung

Rezidivhernien nach einer vorausgegangenen Operation mit Verwendung eines Netzes, eine große Bauchwandhernie mit einer Defektgröße von ≥10 cm oder einem Verlust des Heimatrechts („loss of domain“) von ≥20 %, eine parastomale Hernie und ein übergewichtiger Patient stellen Kriterien dar, die die Definition einer komplexen Bauchwandhernie erfüllen. Leitlinien stellen auf der Basis der vorhandenen Evidenz fest, dass die laparoskopische Reparation von Rezidiven nach vorausgegangener offener Bauchwand- und Narbenhernienoperation empfohlen werden kann. Es liegen keine Studien oder Fallserien zur erneuten laparoskopischen Reparation von Rezidivbauchwand- und Narbenhernien nach laparoskopischer Operation vor. Zur Behandlung einer parastomalen Hernie stellt die modifizierte laparoskopische Sugarbaker-Technik die effektivste Methode dar. Die Ergebnisse der laparoskopischen Reparation von parastomalen Hernien sollten jedoch nicht mit den allgemeinen Ergebnissen der laparoskopischen Bauchwandhernienchirurgie verglichen werden, da die Rezidiv- und Morbiditätsraten höher sind.
