32. Netztechnologie in der Versorgung primärer Bauchwand- und Narbenhernien

Zusammenfassung

Das optimale Netz zur Verwendung bei der laparoskopischen Reparation von Bauchwand- und Narbenhernien sollte eine exzellente Gewebeintegration, minimale Verwachsungsbildung, minimale Schrumpfung, keine Infektion oder Fistelbildung, minimale Schmerzindikation, minimale Seromproduktion, keine Änderung der Biomechanik der Bauchwand und eine einfache Handhabung aufweisen. Deshalb kommen reine Polypropylen- und Polyesternetze nicht infrage. Composite-Netze mit einer resorbierbaren oder nicht resorbierbaren Schutzschicht, PVDF- und ePTFE-Netze werden allgemein für den intraperitonealen Einsatz empfohlen. Biologische Netze stellen vor allem bei kontaminierten Situationen eine mögliche Alternative dar. Aber die sehr hohen Kosten der biologischen Netze und der fehlende Nachweis ihrer besseren Effizienz limitieren deren Einsatz.
