15. Netztechnologie in der Leistenhernienversorgung

Zusammenfassung

In der laparoendoskopischen Leistenhernienchirurgie kommen ganz überwiegend nicht resorbierbare synthetische Netze zum Einsatz. Resorbierbare synthetische und biologische Netze bleiben ausgewählten Fällen vorbehalten. Anhand der verfügbaren Literatur lässt sich bisher nicht sicher belegen, dass die leichtgewichtigen, großporigen Netze gesicherte Vorteile gegenüber den schwergewichtigen, kleinporigen Netzen aufweisen. Die Netzgröße sollte mindestens 15×1 cm betragen. Bei direkten Leistenhernien größer als 3–4 cm und indirekten Leistenhernien größer als 4–5 cm sollte sogar eine Netzgröße von 17×12 cm verwendet werden. Auch geschlitzte Netze sollten nicht zum Einsatz kommen. Falls eine Netzfixierung notwendig ist, sollte sie atraumatisch mittels Klebung erfolgen. Dadurch soll das Risiko für akute und chronische postoperative Schmerzen reduziert werden. Noch besser ist der gänzliche Verzicht auf eine Netzfixierung, was auf jeden Fall bei medialen und lateralen Defekten bis 3 cm möglich ist. Die Rolle der selbstfixierenden Netze muss anhand von weiteren Studien geklärt werden.
