18. Kosten der laparoendoskopischen Leistenhernienversorgung

Zusammenfassung

Die laparoendoskopische Leistenhernienoperation gilt als eine teure Operation und somit in vielen Ländern mit begrenzten finanziellen Ressourcen als nicht durchführbar. Nicht zuletzt aufgrund der riesigen Patientenzahl, die jedes Jahr weltweit am Leistenbruch operiert werden (ca. 20 Millionen), ist eine Analyse der realen Kosten von essenzieller Bedeutung. Drei Aspekte sind zu unterscheiden: 1. direkte Kosten (für die Institution, in der die Operation durchgeführt wird), 2. indirekte Kosten für die Gesellschaft, die zum Beispiel durch die Zeit des Arbeitsausfalls entstehen und 3. die Erlöse, welche die Institution für ihre Leistungen erzielen möchte. In der Leistenhernienchirurgie sind die sozioökonomischen Faktoren höchst relevant. Die Studien, die sich mit den Kosten beschäftigen, empfehlen eine laparoendoskopische Operation, da die Dauer der Arbeitsunfähigkeit bei dieser Technik signifikant kürzer ist als in der traditionellen offenen Chirurgie. In diesem Kapitel werden praktische Tipps im Umgang mit den Kosten gegeben sowie die Ergebnisse der aktuellen Literatur diskutiert.
